Position Overview
The Community Outreach Coordinator plays a pivotal role in conducting health assessments, providing wellness education, and fostering partnerships with senior and community centers. This position combines clinical nursing expertise with outreach and advocacy to improve the health of underserved populations.
Key Responsibilities
- Perform and document blood pressure, blood sugar, and cholesterol screenings. Compare results over time to identify health trends and recommend follow-up care.
- Build and sustain partnerships with senior and community center staff, acting as a liaison to coordinate health-related activities.
- Administer seasonal flu and other vaccines, complete necessary documentation, and ensure timely communication with relevant entities.
- Provide personalized health education and follow-up plans based on screening results.
- Maintain a resource pool of healthcare information and collaborate with business development teams to promote health programs.
- Prepare materials and equipment for screenings, maintain accurate client data, and adhere to infection control protocols.
- Provide reporting on outreach activities to support grant applications and funding opportunities.

Qualifications
Qualifications
- Graduate of an accredited school of professional nursing; BSN preferred.
- Minimum one year of recent experience in acute care or community health.
- Current Missouri and Kansas professional nursing licenses. Current CPR certification.
