Job Summary: We are seeking a dedicated Clinical Manager to ensure coordinated and effective patient care delivery. This role involves supervising clinical personnel, overseeing patient care quality, and maintaining 24-hour accountability for patient care supervision.
Key Responsibilities:
Manage patient case referrals and assign appropriate personnel.
Review and evaluate patient cases through home visits, record reviews, and team conferences.
Assist clinicians in goal setting and care planning.
Supervise and guide clinical staff to ensure effective care delivery.
Participate in new employee screening, hiring, orientation, and training.
Conduct quarterly record reviews and participate in quality improvement initiatives.
Contribute to the development and evaluation of policies, procedures, and education programs.

Qualifications:
Current RN licensure in Missouri and Kansas.
Graduate of an accredited school of nursing; BSN preferred.
Minimum two years of supervisory experience in hospice/home health care.
Strong clinical judgment, communication, and leadership skills.
Valid driver's license and insured vehicle.
Current CPR certification.
