Intake LPN Weekend

Description

We are seeking a compassionate and detail-oriented Intake LPN to support our Home Health intake and referral process. This role involves coordinating with referral sources, evaluating patient eligibility, and ensuring a smooth intake process.

Key Responsibilities:

• Communicate with referral sources to facilitate a timely intake process.
• Evaluate patient eligibility and assist with insurance verification.
• Provide administrative support to clinical staff and field teams.
• Ensure accurate documentation in electronic medical records.
• Participate in performance improvement and professional development activities.

Pay: Starting at $27.30/hr. based one experience

Weekend: Saturday and Sunday

Qualifications

Qualifications:

• Active LPN License (State of Kansas).
• Minimum 1 year of nursing experience (home health experience preferred).
• Strong interpersonal and communication skills.
• Valid driver’s license and reliable transportation.
