- Conduct comprehensive assessments of patients and families to determine their physical, emotional, and psychosocial needs.
- Develop, initiate, and update individualized care plans in collaboration with the interdisciplinary team.
- Provide skilled nursing care, including administering medications, managing symptoms, and offering pain management based on physician orders.
- Educate patients and their families on care techniques, medications, and strategies for coping with end-of-life care.
- Serve as a case manager, coordinating services and resources to ensure continuity of care for assigned patients.
- Maintain accurate and timely documentation of patient assessments, care plans, and interventions.
- Participate in interdisciplinary team meetings and contribute to the overall care strategy.
- Support families emotionally and provide guidance during the grieving process.
