**Job Summary:**
We are looking for a compassionate HHA or CNA to provide personalized care to patients in their homes during their end-of-life journey. The ideal candidate will assist with daily activities, personal care, and emotional support to ensure the comfort and dignity of patients.
**Qualifications:**
- High school diploma or equivalent.
- Massachusetts CNA or HHA Certificate is required.
- Strong communication and interpersonal skills.
**Responsibilities:**
- Assist patients with personal care tasks, such as bathing, grooming, and dressing.
- Maintains strict confidentiality of patient/family/caregiver information at all times
- Maintain a clean and safe environment for patients.
- Communicate effectively with healthcare team members and family caregivers.
- Document care provided and patient progress accurately and timely.
