RN Hospice Clinical Coordinator
**Job Summary:**
We are seeking a compassionate and skilled Hospice Clinical Coordinator to join our team on a full-time basis. The Hospice Clinical Coordinator will be responsible for overseeing and coordinating all clinical activities within our hospice program to ensure high-quality care for patients.
**Qualifications:**
- Bachelor's degree in Nursing or related field required; Master's degree preferred
- Active RN license in the state of practice
- Minimum of 3 years of experience in hospice care
- Strong knowledge of hospice regulations and guidelines
- Excellent communication and interpersonal skills
- Ability to lead and mentor clinical staff
- Detail-oriented and organized
**Responsibilities:**
- Coordinate and manage all clinical aspects of the hospice program
- Supervise and support clinical staff, including nurses and aides
- Ensure compliance with all regulations and standards of practice
- Develop and implement care plans for patients in collaboration with the interdisciplinary team
- Conduct regular assessments of patients' needs and update care plans as necessary
- Provide education and support to patients and families
- Participate in quality improvement initiatives and performance improvement activities
- Collaborate with other healthcare professionals to provide holistic care to patients
- Maintain accurate and complete patient records
