About the Role:
The Registered Nurse will be responsible for providing high-quality, patient-centered care to individuals in their homes. This will involve assessing patients' needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure seamless care coordination.
Responsibilities:
- Provide comprehensive nursing care to patients in their homes, including medication management, wound care, and education on disease management.
- Conduct thorough assessments of patients' physical, emotional, and social needs, and develop individualized care plans to address these needs.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care coordination.
- Communicate effectively with patients, families, and healthcare teams to ensure that care is patient-centered and responsive to their needs.
- Document patient care and progress in a timely and accurate manner, using electronic medical records.
Requirements:
- Current RN licensure in the State of New Jersey.
- At least 1 year of experience in home health care or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
