VNA & Hospice of the Southwest Region of Rutland, VT is looking to hire a part-time Weekend Licensed Nursing Assistant for our Care Plus program. Education
- Valid State of Vermont Nursing Assistant License
Qualifications
- Scheduled work day is Saturday & Sunday, 12 hour days
- High School Diploma or GED
- Valid Driver's License and Current Auto Insurance
- Must be at least 18 years of age
Responsibilities include but are not limited to:
- Under the supervision of a Registered Nurse and/or licensed Therapist, receives daily patient assignments and carries out assignments in accordance with individualized patient plan of care.
- Collects patient and family data and reports findings to RN and/or licensed Therapist in a timely manner.
- Assists in the communication of information to assigned patients.
- Under the supervision of an RN and/or licensed Therapist, performs interventions and procedures as directed to ensure the safe effective quality of care for patients.
- Reports to RN and/or licensed Therapist on a variety of aspects of patient and family information including.
- Advocates for patient care issues to ensure that overall quality and type of care is sensitive to each specific patient/family's needs.
- Performs a variety of patient care tasks including, but not limited to activities of daily living, collection of vital signs, ambulation/exercises, etc.
- Serves as Hospice Aide as directed.
- Serves as Personal Care Attendant as directed.
- Proactively reports changes in skin conditions to RN, Case Manager or program manager in a timely manner.
