Responsibilities
- Assesses and evaluates the needs of patients with life threatening and life limiting illnesses and their families and develops an appropriate plan of care
- Identifies teaching needs and plans appropriate strategies for individuals and families
- Monitors quality and appropriateness of services provided
- Collaborates with patient and family, other members of the disciplinary team and other health care providers in providing patient and family care
- Documents all changes in patient’s condition and updates care plans and outcomes
- Adheres to agency infection control and safety policies
- Adheres to the organization’s policy in regards to absenteeism and appearance
- Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position
- NJ RN license (BSN preferred)
- Certification in Hospice Palliative Care Nurse and Critical Care and Critical Care skills preferred
- 1 year of supervised clinical experience as a registered nurse
- Excellent communication skills
- Strong Computer skills
- Excellent organizational and interpersonal skills
