The Visiting Nurse Association of Florida (VNA) is a 45-year-old home health agency that is growing and looking to hire a two Care Transitional Coordinator (RN or LPN) to work in the Citrus county area.
Summary:
Location: Monday-Friday at TGH Crystal River
Develop relationships with referral sources and clinically coordinate patient care through transition from inpatient to home care setting. Assist sales team in increasing admissions. Serves as liaison between this agency and physicians, hospitals, nursing homes, and other facilities in the medical community. Makes pre-discharge evaluation visits to facilities. These visits are done at the physician's or facility request and it is the admissions coordinator's responsibility to spend time with the patient to promote and explain services the patient is to receive after discharge from the hospital, and to evaluate the patient/family for appropriateness for home care.
