SUMMARY
Coordinates assistance to the Residents in a manner that meets or exceeds community expectations. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to Resident's needs. Provides assistance to Residents with Activities of Daily Living.
ESSENTIAL SKILLS AND RESPONSIBILITIES
1. Complies with facility policies and procedures
2. Supervise and provide assistance (or reminders) with activities of daily living (ADL's). Assist resident in mobility and transfers.
3. Documents care provided, noting unusual or significant changes in physical, mental or behavioral conditions and report changes to supervisor immediately.
4. Assists Residents with medication administration, assistance with Self-Administration of medication, and document administrations on the MOR as directed by the Nurse Manager. Report any miss-administration or refusals promptly to the Nurse Manager.
5. Encourage Residents to participate in residence activities and social programs. Encourages independence to Residents. Comply with all Resident's rights and confidentiality.
6. Responds quickly to emergency calls and requests received via pager or walkie talkie.
7. Provide assistance to Resident's to the activities room / dining room / transportation, etc. Stay in dining room, complete meal attendance checklist and deliver meal trays as needed.
8. Provide effective communication to caregiver at the adjacent shift to encourage a continuum of care.
9. Assists the Activities Manager with scheduled activities.
10. Perform other duties and tasks as assigned or requested.
REQUIRED SKILLS AND ABILITIES
Must be punctual, attendance satisfactory; provide proper notice of tardiness or absences. Attends meetings as required. Must follow facility dress code; maintain good personal hygiene and grooming. Must be mature and emotionally stable; use a calm, kind tone of voice with Residents, staff and family members. Must be able to prioritize and organize work efficiently to accomplish workload within time
Allotted. Must be adaptable and flexible to changing situations. Must be compassionate toward the elderly, and work with tact and ethical awareness, ensure personal Resident information is kept confidential. Look for opportunities to exceed Resident expectations and enhance Resident satisfaction. Must work as a team member. Is willing to help others, assist with maintaining a clean work and storage area. Must maintain a positive attitude. Must respond appropriately to safety hazards, fire drills and emergency situations. Must follow safety rules and regulations. Report injuries / accidents to Administrator immediately per facility procedures. Supports continuous improvement. Look for ways to reduce costs and create efficiencies on a daily basis. Perform other duties as assigned.
QUALIFICATIONS

EDUCATION AND EXPERIENCE
High school education / GED; or equivalent combination of education and experience. (Maintain current CPR and First Aid Training). Level 1 and Level 2 Alzheimer certified, Abuse, ADL, Adverse and Major Incident Reporting, Behavior and Needs of Resident, DNRO, Elopement, Facility Emergency Procedures, HIV / AIDS Infection Control. Universal Precautions, Facility Sanitation Procedures, and Resident Rights Training.
LANGUAGE SKIILS
Ability to read and comprehend simple instruction. Ability to interpret medical directions and medication measurements. Able to document all assignments thoroughly including medication administration.
MATHEMATICAL SKILLS
Ability to multiply and divide by 2's, 5's and 10's. Able to use all liquid measurements. Ability to use weight measurements.
REASONING ABILITY
Demonstrates medical assessment skills. Able to make decisions to address simple variations to standards. Able to make appropriate decisions of when to for assistance.
CERTIFICATES, LICENSES, AND REGISTRATIONS
Assistance with Self-Administration of Medications.
PHYSICAL DEMANDS
While preforming the duties of the job, the employee is regularly required to stand; walk; use hands to finger, handle of feel; reach with hands and arms; climb or balance; stoop, kneel, crouch; talk or hear; and smell. The employee is occasionally required to sit. The employee must regularly lift and / or move up to 100 pounds and occasionally lift and / or move up to 200 pounds or more. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus.
