Your primary responsibility will be to provide high-quality nursing care to patients and families, working under moderate supervision. You will assess patients' physical, social, and psychological needs, develop individualized care plans, and provide education and support to patients and families.
In addition to your clinical responsibilities, you will also participate in interdisciplinary team meetings, collaborate with other healthcare professionals to ensure comprehensive care, and monitor patients' progress and adjust care plans as needed.
Key Qualifications:
- Licenses and Certifications: License to practice as a Registered Professional Nurse in New York State required
- Education: Associate's Degree in Nursing or Bachelor's Degree in a health or human services field
- Work Experience: Minimum of one year of clinical nursing experience, including at least six months of Hospice or other related experience
