Job Responsibilities
- Collaborate with clinicians to implement best practices in documentation and OASIS completion.
- Develop training activities with other departments to address clinician learning needs.
- Provide coaching, education, and performance evaluations to clinicians.
- Lead special projects to improve outcomes and efficiency in patient care.
Requirements
- License to practice as a Registered Professional Nurse, Physical Therapist, Occupational Therapist, or Speech Language Pathologist in New York State.
- OASIS certification within one year of job entry date.
- Bachelor's Degree or Master's Degree required.
- Minimum three years of clinical experience in community healthcare.
