Key Responsibilities
- Evaluation Documentation Review: Ensure accurate completion and approval of evaluation documentation, OASIS assessments, and Patient Plans of Care.
- Data Verification: Verify data submitted from field staff for accuracy and follow up on any documentation requiring correction.
- OASIS Processing: Process OASIS and Patient Plan of Care, ensuring correct start of care dates and episode date ranges.
- Trend Identification and Escalation: Identify and escalate trends to leadership.
Qualifications and Requirements
- Current registration to practice as a Registered Professional Nurse, Physical Therapist, Occupational Therapist, or Speech Language Pathologist in New York State.
- OASIS certification within one year of job entry date.
- Bachelor's Degree or Master's Degree required.
- Minimum three years of clinical experience in community healthcare.
