Job Summary
This position involves working as a Registered Nurse Field Supervisor, providing critical care coordination and supervision to Home Health Aides in the field. Responsibilities
- Develop and implement individualized care plans to meet the unique needs of patients.
- Conduct regular supervisory visits to patients' homes to assess their condition, provide training, and coordinate care.
- Collaborate with the clinical care team to ensure seamless care transitions and effective communication.
- Supervise and orient Home Health Aides to ensure they possess the necessary skills and knowledge to provide high-quality care.
Requirements
- Bachelor's degree in Nursing.
- Current NY State RN licensure.
