Job Responsibilities Include:
Evaluates the patient’s ability to function in the home and determines the need for therapeutic interventions, appliances and or modifications of the environment.
Develops a therapeutic plan of treatment and appropriate goals in collaboration with the patient, family, physician and other health team members.
Provides direct care to treat disabilities and injuries through the use of accepted occupational therapy modalities.
Identifies and reports significant clinical information to the physician, Clinical Manager and Case Manager in coordinating services.
Documents all clinical evaluations, treatments and patient’s responses to therapy interventions. Instructs and supervises the patient and caregivers in the home exercise program and other aspects of patient care as appropriate and documents their response to instruction.
Systematically plans for discharge in collaboration with the patient, family, physician, Clinical Manager and interdisciplinary team.
Qualifications:6. Valid driver’s license.
Job Types: Full-time, Part-time
Pay: $80,000.00 - $100,800.00 per year
Expected hours: 21 per week

- NYS Professional License and current registration to practice as an occupational therapist
- Graduate of an Occupational Therapy program approved by the New York Board for Occupational Therapy or AOTA
- One (1) year of full-time experience in direct provision of occupational therapy services
- Home care experience preferred
- Automobile in good working order and auto insurance in the amount specified by VNSW
