The Performance Improvement Specialist is responsible for the guidance of best-practice care planning and optimal utilization of services; provides episode management education to multi-disciplinary teams in the management of patient care services. The Performance Improvement Specialist reviews quality metrics, utilization statistics and adverse events, including the study of hospitalized patients; seeking trends and providing correlating opportunities to mitigate preventable risk; setting ongoing expectations for standardized, risk-stratified, best practice care planning.

Responsibilities:
Clinical Professional such as a Registered Professional Nurse or Physical Therapist.
Bachelor’s Degree in discipline required, Master’s Degree preferred.
Minimum Of 4 Years Of Home Care Experience.
OASIS Certification required.
Previous leadership experience desired.
Flexible in scheduling and hours worked and innovative.
Consistently seeks resources to educate self and others on evidence based best practices initiatives (such as methodology to reduce avoidable hospitalization and improve specific clinical outcome scores.)
Ability to summarize and report on trends and outcomes.

Responsibilities:
- Conducts episode management meetings with the interdisciplinary teams, using a grand rounds approach to illustrate expectations for standardized use of risk-stratified and best practice care planning
- Collaborates with Clinical Operations, Quality Assurance and Performance Improvement leadership and Integrated Care Management to continuously evolve risk-stratified clinical practice guidelines. Through the Episode Management meetings, conveys standards using patient examples to illustrate the application of best practice, illustrating opportunities to prevent avoidable hospitalization or other adverse events through application of evolving standards of care
- Recommends additional services, or techniques to improve efficiency and effectiveness of cases presented in Episode Management meetings; illustrating optimal, risk-stratified Plan of Care and to achieve optimal patient outcomes
- Monitors quality metrics and utilization patterns to identify clinician, team or agency trends related to patient outcomes and high or low visit utilization, relative to benchmark data. Integrates optimal visit utilization expectations, based on clinical acuity and associated management of risk, into Episode Management meeting discussion and conveyed expectations of practice
- Participates in the training and education of new clinical staff related to care planning and OASIS accuracy
- Promotes a cooperative, cohesive group process dedicated to provision of quality patient care with achievement of best possible outcomes, provides education and coaching to VNSW clinical staff
- In collaboration with the Clinical Manager, develops and delivers performance improvement plans as part of corrective action for clinical staff
- Monitors and maintains utilization statistics in line with national best practice benchmarks, working to optimize clinical outcome scores as evidenced by Home Health Compare and its equivalents
- Actively participates in performance Improvement activities within the location/region/service line
- Collaborates and strategizes with the Quality Team and location leaders to improve patient outcomes
- Identifies patterns and issues for performance improvement and provides timely feedback to clinicians, Clinical Managers, Administrators and Quality Department, as necessary
- Maintains knowledge of federal and state regulations and accreditation standards as well as agency policies and procedures
- Reviews and approves Plan of Care and Physician Orders; reviews and approves OASIS assessments
- Conducts various focused audits including but not limited to: Transition of Care Audits, Potentially Avoidable Event Audits, Plan of Care Audits, using data to identify trends of risk- patterns, and areas for improvement. Applies trended findings to selection of case study and ongoing clinical team education; setting new expectations for care planning
- Maintains effective and positive communication with field staff, Clinical Managers, Administrators, and other departments
- Relates with tact and respect at all times to internal and external customers. Actively builds positive relationships with internal and external customers
- Responds to requests for additional information within established time frames
- Facilitates cross service line collaboration and strengthens clinicians’ problem solving to support continuity of care
- Services as a clinical resource to the clinical staff and quality department
- Provides education to clinical staff and location leadership to evidence-based practices to achieve optimal patient outcomes
- Gathers quarterly quality data and prepares reports for committee and Board meetings
- Supports adherence to agency policies and procedures in regards to Episode Management
