Key Responsibilities:
- Deliver high-quality patient care as a Home Health RN in a dynamic home health setting
- Develop and maintain strong relationships with patients, families, and healthcare teams
- Collaborate with multidisciplinary teams to ensure seamless patient care transitions
- Maintain accurate and detailed patient records, adhering to industry standards and regulations
Requirements:
- Minimum 2 years of recent experience as a Home Health RN
- Valid certification and/or licensure in the state of practice, if applicable

- Valid Driver's License and ability to travel between assignment locations
- Ability to pass a background check and drug screen to meet healthcare industry standards
