- Provide direct patient care to residents in long term care settings.
- Develop and implement individualized care plans tailored to each resident's unique needs and goals.
- Collaborate with interdisciplinary teams to ensure seamless communication and coordination of care.
- Maintain accurate and timely documentation of patient progress and outcomes.
Requirements:
- Minimum of 2 years of recent experience as a Long Term Care RN.
- Demonstrated ability to maintain a high level of professionalism and composure, especially during stressful and fast-paced moments in the healthcare setting.
- Valid Driver's License and the ability to travel between assignment locations.
- Ability to pass a background check and drug screen to meet healthcare industry standards.
