What are the responsibilities and job description for the Co-Occurring Care Manager position at Volunteer Behavioral Health Care System?
Volunteer Behavioral Health-Chattanooga Campus has an immediate opening for a Full-Time Co-Occurring Care Manager.
Co-Occurring Care Management Duties:
- Engage clients in a treatment setting by developing supportive activities aligned with recovery topics.
- Provide care management to adults and children focusing on strengths of individuals and families. Care management services assist individuals in gaining access to and maximizing the benefits of needed medical, social, educational and other support services.
- Direct weekly face-to-face client care with encouragement and guidance to support recovery from co-occurring disorders. Regularly assess strengths, needs, and barriers of each client. Work with clients on individual barriers to ensure engagement in treatment. Provide resources to help clients engage in treatment and long-term recovery. Work alongside IOP Facilitators to discuss the level of care needed for clients based on engagement in treatment. Assist in transition to higher level of care and/or additional sober living supports. Develop integrated treatment plans, aftercare, transition, relapse prevention and discharge plans with clients.
- Assess for need and assist with scheduling services outside of IOP (Vivitrol injections, therapy appointments, medication appointments, and intake evaluations.) Provided ongoing support in reviewing any barriers to treatment to ensure compliance with external services.
- Attend support group meetings alongside clients to assist with barriers to developing sober community support.
- Suicide safety planning and referrals to crisis stabilization unit.
- Manage incoming referrals from multiple agency referral sources to include weekly follow-up, continued client compliance and ensure treatment goals are aligned.
- Conduct weekly orientation to provide new clients with program requirements, treatment goals, crisis planning, and familiarize with office set up and schedules of groups.
- Administer urine drug screens as needed for client’s compliance with treatment and/or referral source requirements.
- Uphold company standards for completing ECT required documentation. Regularly audited files to ensure successful quality improvement audits.
- Maintain spreadsheets to regulate the program and ensure timely delivery of services.
- Maintain state of Tennessee grant-based computer programs such as TN-WITS for ASI and ASAM assessments.
- Manage Vivitrol grant clients to ensure that they have had their injections every 28 days to prevent relapse.
- Perform as an integral part of the development of program standards. Work as part of a team to develop graduation program requirements.
- Assist other agency locations with training new care managers in IOP program.
- Assist with grant billing each month based on client attendance and services participated in.
- Facilitate referral outreach to community partners, current and potential referral sauces.
- Ensure appropriate document management.
- Complete monthly quality assurance reviews of program requirements as determined by the MCO’s and State.
The Care Manager is expected to follow guidelines and maintain a level of performance consistent with current productivity expectations as stated in attachments A and B of the job description.
EDUCATION / EXPERIENCE
Must have a Bachelor’s degree in a health-related field of counseling, psychology, social work or other behavioral sciences.
