HOMELESS OUTREACH CASE MANAGER (PATH Program):
Immediate full-time PATH Coordinator position opening at our Cookeville Campus.
This position is responsible for obtaining program resources, all reports/record keeping, assessment to determine eligibility, and paperwork involved with the PATH Grant. The program focuses on working with adults with a mental illness and homeless, providing daily living assistance, and referrals to community resources, advocacy and transition to mental health services.
QUALIFICATIONS:
A bachelor's degree in psychology, Social Work, or related area is required.
Experience working with the homeless preferred and must have an understanding of homelessness.
Job Type: Full-time
Pay: From $38,000.00 per year
