Description
Coordinator responsibilities include, but are not limited to:
- Direct oversight of the PATH Program.
- Oversight and training of new program staff.
- Supervision of all program staff.
- Screening and assigning referrals.
- Network and attend meetings with other agencies to include coalitions, homeless providers, and social service providers.
- Establish weekly meetings with Homeless Outreach Case Managers.
- Maintain supervision log for weekly meetings with Homeless Outreach Case Managers.
- Management of program budget.
- Management of Rutherford County homeless count by working closely with internal and external referral sources to ensure program access.
- Coordinating community donation.
- Reports/Record keeping:
- Complete state and federal reporting requirements and turn in all monthly paperwork as scheduled.
- Responsible for signing all time sheets, mileage reports and reimbursement forms of Homeless Outreach Case Managers.
- Complete and turn in all mileage reports and reimbursement forms by the required deadline.
Case Management duties include, but are not limited to:
- Maintain a caseload and management of that caseload.
- Understand and adhere to the scope of services.
- Work with internal and external referral sources to ensure program access.
- Assist with screening of referrals.
- Work with consumers to meet their mental health and housing needs.
Qualifications
Minimum of a Bachelor’s Degree in social work, psychology or related field required.
The skills and general abilities required to perform these duties are typically acquired through experience working in the community setting with adults who have a severe and persistent mental disability and must have an understanding of psychotropic medication and the medication effects upon the user.
Experience with the homeless population helpful.
Salary : $38,000
