JOB SUMMARY
The Case Manager is directly responsible for the service provision and housing assistance for the residents at Freedom Springs, a permanent supportive housing property for Veterans located in Colorado Springs, Colorado. This position works in conjunction with the Supervisory Case Manager, an external property management company, local coordinated entry system providers, and the project developer. This program provides supportive services to Veteran individuals and families. Service modalities include Housing First, Trauma Informed Care, Harm Reduction, Motivational Interviewing and Critical Time Intervention.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Responsible for identifying and serving the unique needs of participant households participating in Volunteers of America programs by utilizing screening and assessment tools and ensuring the completion of needs-based screenings and independent living assessments.
- Assists in relocating residents as needed and securing emergency housing as needed.
- Provides tenant/property management mediation as needed.
- Attends orientations and lease-ups with property management and landlords as appropriate.
- Develops and maintains a good working relationship with community partners including property management and project development staff.
- Responsible for gaining an in-depth knowledge regarding program requirements, including the referral process, support services, allowable costs, field-relevant best practices, utilization of the coordinated entry system and HUD permanent supportive housing standards.
- Assists with community-wide outreach to identify eligible participant households presenting with substantial barriers to housing stability.
- Assists residents filling out rental applications, interpreting leases and understanding tenant rights and responsibilities.
- Responsible for assisting participants in obtaining necessary documents to obtain and maintain housing including identification documents, income changes, changes in household composition, etc.
- Develops and coordinates health, wellness, educational, or any other event at the property that would be beneficial to the independent living of Freedom Springs residents.
- Plans recreational outings and activities for Freedom Springs residents.
- Assists in creation of monthly newsletter published for all residents.
- Attends orientations, trainings, education programs, staff meetings, community meetings, conferences, and workshops as requested and applicable to meet the needs of the position.
- Performs duties in a professional manner by maintaining the confidentially of all information and by participating effectively within and across teams.
- Performs all other duties as assigned.

- Knowledge and skill in the application of Harm Reduction, Critical Time Intervention, Motivational Interviewing, Trauma Informed Care, and Housing First Principles.

Requirements
MINIMUM QUALIFICATIONS
- Bachelor’s degree in human services, social work, or a closely related field or related experience and 1 year of direct, full-time case management experience or a Master’s degree in social work, counseling, psychology, or a closely related field.
- Possess a Colorado driver’s license and state-mandated automobile insurance.
- Possess a personal vehicle that may be used for work-related travel (reimbursement for mileage is available)-No resident transportation permitted.
- Complete agency and program credentialing within 12 weeks of hire and maintain credentialing standards thereafter.
- Experience working and providing services in permanent supportive housing environment.
- Experience working with vulnerable populations, including individuals experiencing homelessness and the Veteran population preferred.
- Experience working and providing services in permanent supportive housing environment preferred.
