Position: Nurse-RN (Part Time)
In accordance with Division of Mental Health and Addictions Services (DMHAS), Division of Developmental Disabilities (DDD), Commission on Accreditation of Rehabilitation Facilities (CARF), Medicaid, and Volunteers of America policies and standards, the registered nurse is responsible for the direction, provision, and quality of the health maintenance and monitoring services provided to the service recipients and staff, throughout the sector.
Community Support Services consists of 10 programs that provide behavioral health rehabilitation services that are necessary to assist people in achieving recovery goals such as employment, education, housing, and/or social integration. Clients participating in CSS have the right to choose their housing and the services they need. These services are provided within a framework that promotes psychiatric rehabilitation, wellness, and recovery. We currently assist 170 individuals in scattered site apartments throughout Passaic, Middlesex, and Union Counties. Minimum Qualifications:
This position requires a license from the State of New Jersey as a Registered Nurse and relevant experience.
This position shall also have a valid state driver’s license with a safe driving record that meets agency procedures.
Nurse-RN Principal Responsibilities:
Assess the health needs of assigned service recipients and make appropriate medical referrals as needed.
Maintain documentation according to the Division of Mental Health and Addictions Services (DMHAS), Commission on Accreditation of Rehabilitation Facilities (CARF), Medicaid, and Volunteers of America policies and standards.
Sign-off on and participate in the development of the Individual Service Plan (ISP).
Support maintenance of compliance with OSHA standards.
Provide education and consultation on medication and health maintenance issues.
Maintain service recipient contact hours per Division of Mental Health and Addictions Services (DMHAS) and Medicaid standards.
Provide and document face-to-face health care monitoring for the initial assessment in the first 14 days of admission, ongoing assessments every 90 days, and re-assessments annually.
Provision of 24/7 on-call consultation and support.
