SUMMARY
Under direction, the SSVF Service Coordinator shall provide case management services and support to eligible program participants; records and maintains records and documentation as required by the program; performs related duties as assigned.
ESSENTIAL JOB FUNCTIONS
- Provide case management services and assist program participants in accessing supportive services and benefits to promote housing stability.
- Screen and determine program eligibility.
- Conduct a needs assessment with each participant; develop housing stability plans and goals with participants.
- Maintain a caseload of program-eligible participants and establish constructive relationships with them.
- Conduct periodic evaluations and provide regular feedback regarding client’s housing search and service coordination with team members and supervisor.
- Record all program information in the applicable database; maintain file records and documentation as required by the program.
- Conduct outreach in the community, including street outreach to homeless veterans and families.
- Establish relationships with landlords and property management companies for the benefit of program participants.
- Attend community meetings, BNL, and participate in presentations about SSVF program.
- Attend professional development training and activities.
- Maintain and update knowledge through conferences, in-service trainings and seminars.
- Must have skills in the MS Office 2000 Suite or higher (includes WORD, EXCEL, POWERPOINT, EMAIL OUTLOOK and Internet Explorer).
- Promote positive image of VOA in all communications internal and external to work settings
Additional Responsibilities
- Represent VOAMI in a professional manner when conducting activities in the community and attending community events with community partners.
- Maintain compliance with VOA Michigan policies and procedures
- Ensure that all duties are carried out in a timely and orderly fashion so that VOA Michigan functions in the most efficient manner possible
- Refer all SSVF participants into SSVF funded legal services program (legal sub-grant)
- Complete all relevant job-related trainings both on and off of VOAMI’s Relias system in a timely manner
- Must maintain a valid Michigan Driver’s License and an operable insured automobile for authorized travel
- Must have a driving record that is acceptable to VOAMI’s insurer
- Other duties as assigned
REQUIRED KNOWLEDGE SKILLS, AND ABILITIES
- Bachelor’s degree in social work, human services, related field or equivalent of 3 years’ experience with veterans, homeless populations and/or disadvantaged population.
- Equivalent combination of education and experience to include training received while serving in the military.
- Ability to establish effective relationships with staff, co-workers, outside agencies, and the public.
- Ability to work independently and meet deadlines with minimal supervision.
- Strong interpersonal skills to build relationships with employers and manage employer accounts
- Availability to work a non-traditional schedule that includes holidays, evenings and weekends
- A professional appearance and sensitivity to others
- Strong organizational skills
- Information technology skills
