LPN Health Service Coordinator Job Description
The LPN Health Service Coordinator will assist service recipients with health-related activities that they would manage themselves if able. This role involves providing clerical and office support in maintaining healthcare services, ensuring accurate documentation, and managing medical supplies for individuals with developmental disabilities.
This position requires a current license as a Licensed Practical Nurse in the State of Kentucky and at least two years of nursing experience, preferably working with individuals with intellectual and developmental disabilities or in a home health environment.
Duties and Responsibilities
- Enter health-related information into the practical health record system
- Maintain paper files relative to healthcare
- Coordinate medical appointments and follow-ups
- Follow up on referrals and recommendations from healthcare professionals
- E nsure person-supported records meet DIDD requirements
- Review and sign off on medication variances weekly
- Monitor and maintain medication refill requests and order supplies as needed
- Coordinate AT equipment and repairs
- Filing of medical documents and encounters in charts daily
- Ensure consents are updated and uploaded to person-supported files
- Review, complete, and upload encounters in EHR
- Thinning charts of persons supported and storing them according to office procedure
- Monitor Lab tracking and request copies for files
- Track annual physicals, dentals, and vision exams within DIDD guidelines
- Work in homes at least 16 hours a month or as needed
- Be available and on call when the Case Manager is unavailable
- Coordinate between healthcare professionals to ensure effective information flow
- Perform quality improvement duties and reports as assigned by supervision
- Any other job duties as assigned by supervision
