Pay: $36.00 - $51.33 per hour, includes pay in lieu of benefits option!
Schedule: Friday 8am - 5pm and participation in on-call rotation (evenings and weekends)

Senior Community Care of Colorado (SCCCO) is a PACE Program (Program of All-Inclusive Care of the Elderly) serving older adults in Montrose and Delta Counties. Our team offers wrap-around care services to meet the unique needs of each of our participants and support them to remain living independently in their homes and communities. Required Qualifications:
- Graduate of a school of professional nursing.
- Current Colorado Registered Nurses (R.N.) License, Cardio Pulmonary Resuscitation (CPR) certification required, Bachelors of Science Nursing (BSN) preferred.
- Unencumbered license as a Registered Nurse (RN) in the State of Colorado required.
- Must have a valid driver’s license, proof of insurance and have means of transportation.
- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
- Knowledge about the PACE model and philosophy of care
- Functions as a member of the Care Team. Maintains regular attendance at, and participants in Care Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery. Effectively communicates with team mates during meetings or one-on-one conversations.
- In coordination with the Care Manager and other nursing staff, develops and implements a plan of care to meet the needs of participants to remain living safely in the community for as long as possible in alignment with their goals of care. Maintains aligned, comprehensive plan of care for all participants living in residential facilities and coordinates care between facility and Care/Interdisciplinary Team
- Conducts a comprehensive initial nursing assessment with new PACE enrollees. Coordinates with the Care/Interdisciplinary Team to develop a comprehensive plan of care for each participant. Conducts nursing re-assessment semi-annually and as needed.
- Performs general and skilled nursing care to participants as assigned. Implements the plan of care and physician orders in coordination with the Care/Interdisciplinary Team. Observes, records, and reports participant’s condition and reaction to drugs and treatments to providers. Triages participants to determine priority of care and assists in the treatment, examination, and testing of participants
- Participates in nursing on-call rotation to provide after-hours and weekend assistance and care, as needed, to PACE participants.
- Receives participant complaints, triages, and responds to participant needs.
- Reviews and signs off orders and disseminates orders and instructions to appropriate staff and contracted personnel. Maintains and reviews participant records, charts, and other pertinent information. Completes documentation and charting in accordance with organizational policy.
