Pay: $90,000- $95,000
Monday-Friday 8:00 AM-4:30 PM, on-call if needed
The Clinical Care Manager under the supervision of the Executive Director, oversees and manages the overall clinical effectiveness of Senior Community Care (SCC) clinical services to ensure the efficiency, consistency, continuity, and quality of care provided to program participants. Is accountable for meeting organizational and participant outcome and utilization goals.

- Unencumbered license as a Registered Nurse (RN) in the State employed required
- Proven experience leading a team of professionals.
- A minimum of one year’s experience in working with the frail or elderly population
- Must have a valid driver’s license, proof of insurance and have means of transportation
Preferred Qualifications:
- Bachelor’s or Master’s Degree from an accredited school in nursing strongly preferred.
- A minimum of five (5) years of nurse management experience in a hospital, nursing home or community-based setting strongly preferred
- Bachelor’s or Master’s Degree from an accredited school in nursing strongly preferred.
Essentials:
- Provides oversight of the overall effectiveness of clinical care provided to SCC participants across all care settings and is responsible to meet defined organizational participant outcome, utilization, and quality goals.
- Provides oversight, management, and coordination of the consistency and quality of the twenty-four (24) hour care provided to program participants across all care settings.
- Provides oversight, management, and direction of the overall operational effectiveness of PACE clinics and is responsible for effectuating and maintaining effective and efficient operational systems, processes, and workflows in compliance with applicable regulations and organizational policies to meet participant care needs and organizational objectives. This includes, but is not limited to, staffing schedules, inventory management and procurement, policies and procedures, clinical workflows, etc.
- Provides direct supervision and support of the Care Managers, Clinic Coordinators, Nursing personnel, Registered Dieticians, Nurse Practitioners, and Rehab Manager.
- Engages in team building. Coaches and mentors staff. Instructs and teaches staff as needed. Trains clinical staff to function independently. Ensures that the job responsibilities, authorities and accountability of direct reports are defined and understood. Assists in the establishment and enforcement of organizational policies and procedures.
- Responsible, in coordination with the clinical team and IDT, for utilization management including minimizing unnecessary or preventable utilization, safe and effective discharges and transitions between care settings, and utilization review and improvement efforts to meet participant care needs.
- In conjunction with the Care Managers, acts as the primary liaison between program and contracted providers and facilities for the coordination of effective care and treatment of program participants.
