Senior Community Care of Colorado PACE is part of Volunteers of America National Services which serves as the Housing and Healthcare subsidiary of the Volunteers of America parent organization.
Pay: $70-$75k annually
Schedule: Monday-Friday 8am-5pm with occasional weekends

The Community Outreach Nurse (RN) supports and assists with promoting and growing our PACE ministry through community outreach and the enrollment intake process.
QUALIFICATIONS:
- Must have a valid driver’s license, proof of insurance and have means of transportation.
- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
- Graduate of an accredited school or program of nursing.
- Licensed Registered Nurse (RN) in the State of Colorado.
- A minimum of one year’s experience working with frail or elderly population required.
Skills and Knowledge:
- Ability to read, speak and write fluently in English.
- Excellent interpersonal communication and phone skills.
- Ability and willingness to work in various locations and environments.
- Comfortable with speaking to individuals and groups. Strong public speaking skills.
- Excellent customer-service orientation.
- Ability and willingness to work in a diverse team environment.
ESSENTIAL FUNCTIONS:
- Receives referrals from the community and follows up with the Intake Specialist
- Completes the initial PMIP for potential participants. Completes home safety evaluations and identifies the immediate service needs. Presents this information to the Enrollment Determination Team.
- Participates in healthcare-focused community outreach opportunities, events, health fairs, community meetings, and activities on behalf of the PACE organization. Assists in establishing Senior CommUnity Care of Colorado’s (SCCCO) presence in the healthcare community by outreaching virtually, telephonically and in-person.
- Assists and supports with Provider orientation / reorientation.
- Completes level of care documentation for functional re-certification in Montrose and Delta Counties.
- Acts as a liaison with Care Teams and IDT for new enrollees.
- Works to strengthen relationships with key community partners and stakeholders, including contracted providers, community organizations, government agencies, and more.
- Supports the management of community relationships, provides continuing education, and participates in provider orientation/reorientation.
- Maintains accountability for high customer service standards and monitors, evaluates, and recommends opportunities for improved participant and family/caregiver satisfaction.
- Responsible for adherence to SCCCO’s Standards of Service Excellence including supporting the development and integration of these standards within the team. Champions the mission of SCCCO.
- Maintains the stability and reputation of SCC by ensuring all activities and operations are performed in compliance with local, State, and Federal laws, regulations, and contractual requirements.
- Follows all organizational policies and procedures and CDC and Occupational Safety and Health Administration (OSHA) safety guidelines.
- Acts within the scope of his or her authority to practice.
- Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions.
- Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
- Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
- Performs other duties as required or requested.
Senior Community Care of Colorado (SCCCO) is a PACE Program (Program of All-Inclusive Care of the Elderly) serving older adults in Montrose and Delta Counties.
