About the Job:
We are seeking a Nurse Practitioner - Primary Care Specialist to join our team at Senior Community Care of North Carolina. The ideal candidate will have experience working with geriatric populations and be certified as a nurse practitioner by a national certifying organization.
Key Responsibilities:
- Provide primary care to participants in a PACE program
- Perform physical assessments and develop plans of care
- Collaborate with the Interdisciplinary Team to coordinate care
- Refer participants to medical specialists as needed
Requirements:
- Graduate of a school of professional nursing required
- Licensed as Registered Nurse (R.N.) in the State of North Carolina
- Certified as a nurse practitioner by a national certifying organization recognized by the State of North Carolina Board of Nursing
