What are the responsibilities and job description for the Case Manager position at Volunteers of America of Pennsylvania?
Responsibilities: Include, but are not limited to: 1. Understand and use the Recovery Model, Community Support Program Principles, and Supported Living principles while working with consumers with the belief that people will recover. 2. Interview referrals to obtain consumer history, to provide agency information and to determine and verify eligibility for program services. 3. Secure information such as medical, psychiatric, psychological and social factors to better understand their current situation and to evaluate supported living needs. 4. Develop and maintain with the consumer an individualized service plan which includes the consumer’s personal goals. 5. Review, monitor and evaluate service plans to determine consumer’s progress with their goals. 6. Manage caseload, prepare reports and keep well organized and detailed records of services provided. 7. Assist consumer with arranging transportation or provide transportation when needed. 8. Provide support and assistance to the consumer with their pursuit of personal goals. 9. Collaborate with other provider staff involved with the consumer. Work together to assist the consumer with developing and/or maintaining independent living skills and attaining personal goals. 10. Work with the consumer to obtain other community services, resources and supports. Make referrals and provide advocacy for the consumer if necessary. 11. Provide the consumer with the necessary tools which will enable the consumer to pursue services, resources and supports independently. 12. Attend training relevant to position. 13. Other duties that maybe relevant to the position or otherwise assigned. Qualifications: At least one (1) year professional level experience in human services with a minimum (6) months experience in Mental Health. Required: Effective oral and written communication skills; strong organizational and time management skills; keen attention to detail; the ability to develop effective working relationships with clients, staff, and community partners; a professional appearance and sensitivity to vulnerable populations. This position requires driving a company and/or a personal vehicle. The successful candidate must be privileged to drive according to guidelines set forth by the Pennsylvania Department of Transportation and must maintain vehicle liability coverage if using a personal vehicle for client transport.
