Safe Haven is a housing first program intended to service veterans experiencing comorbid disorders (mental health, substance use or chronic medical conditions) with obtaining permanent housing and build skills that are conducive with maintaining that housing. The Case Manager will engage veterans with obtaining permanent housing through comprehensive support services that includes direct one-on-one and group settings. The Case Manager will aid veterans in building skills to assist them with successfully securing and maintaining housing using a client-centered and trauma informed approach. Veterans and case managers work collaboratively to identify needs through completion of assessments, goal plans and follow up. Activities can include assistance with identification of housing options, linkage to community resources, completion of required paperwork, transportation, and tracking of veteran appointments related to housing and healthcare.
The Case Manager facilitates weekly groups focused on topics such as life skills, communication and coping mechanisms. Case management services are provided collaboratively with other program staff, clinical supervisor and external partners.
The Case Manager is required to meet deadlines regarding documentation such as case notes, goal plans and veteran activities utilizing an electronic record system. Compliance with Volunteers of America, Veterans Administration and Commission on Accreditation of Rehabilitation Facilities (CARF) standards is required.

Applicants must have a Bachelor’s degree in the area of human and/or social services or the behavioral sciences. Licensed Social Worker designation is highly preferred. Education and/or verifiable work experience with at-risk populations is required (homeless, veterans, behavioral health, drug/alcohol addiction, employment, youth, residential). First Aid & CPR certification is required and must be maintained. VOAOHIN will provide certification if not already obtained
