The Outreach Worker will be responsible for identifying and providing outreach services to homeless or at-risk Veteran families, residing on the streets, shelters, hunger programs or in places not meant for human habitation. Additional duties will include providing assistance to individuals in immediate need of services such as mental health treatment, drug/alcohol treatment, shelter, food, clothing, and transportation. The incumbent will serve as the initial point of contact to link and utilize available community network systems.

Preferred candidates will have a high school diploma with a minimum of two years experience in a related area.
