Function:
The Case Manager is responsible for providing case management with wrap around support services for homeless veterans. The Case Manager will develop and monitor an individualized service plan with the resident in an effort to employ residents to live more independently and reduce the possibility of future homelessness. He/she will provide assessment, case management, counseling, referral service and life skills training.
Know How:
The position requires a minimum of a Bachelor's degree in a human services field and two years of transferable experience. Experience in client assessment, case management, substance abuse, crisis intervention, and familiarity with available community resources is essential. Position requires the flexibility to working evening and weekend hours. Veterans are encouraged to apply.
Principal Activities:
- Conduct assessments and develop/monitor individualized service plans for program participants enrolled in the Transitional Housing Program.
- Provide counseling and referral services for a variety of issues to include substance abuse, mental health, and family aspects of operation.
- Oversee the ongoing implementation of an effective case management program.
- Supervise the timely implementation of Life Skills Training, including developing pre and post tests, as indicated in the grant application.
- Attend and oversee daily community meetings and the Veterans’ Advisory Council.
- Ensure all required documentation is kept in individual case management files.
- Provide weekly case management evidence.
- Assist with program participant transportation, as needed.
- Case Managers are encouraged to participate in outreach and community service, as indicated in the grant application.
- Represent the interest of the agency, the programs, and the program participants in all networking and public relation opportunities. This includes maintaining a strong working relationship with the Veterans Administration and other veterans/homeless advocacy and service groups.

Language skills:
Ability to read, analyze, and interpret, common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to effectively present information to top management, public groups and/or boards of directors.
