Function:
The Licensed Practical Nurse (LPN) is a support personnel for the Substance Abuse Program. This person is an integral member of the Interdisciplinary Team. The LPN provides nursing services, health care monitoring, and staff training. The LPN will also be responsible for maintaining accurate patient records and communicating with other healthcare professionals to ensure the best possible patient outcomes. The ideal candidate will be computer literate, organized, and a good team player.
Know How:
The position requires a valid LPN licensure. A minimum of one year nursing experience is preferred. This position requires the ability to work closely with other agency personnel and the medical community, knowledge of physical assessments, and knowledge of pharmacology, particularly psychotropic medication. Must possess a valid GA driver’s license and continually meet the requirements for VOASE’s vehicle insurance company.
Principal Activities:
- Evaluate physical condition of individuals utilizing examinations of vital signs and body systems.
- Obtain necessary information regarding individual’s health in an accurate and thorough screening assessment process.
- Obtain and record all prescribed medication(s) in medical section of record of person served.
- Provide, administer and read T.B. test prior to the person served entering the program.
- Work cooperatively and collaboratively with private physicians of persons served, to meet the healthcare needs of individuals as prescribed by their physicians and outlined in Best Practices.
- Obtain all required signed releases of information.
- Drive facility van, transporting persons served to appointments as needed. Coordinate and utilize medical transportation company if there is a schedule conflict with VOASE, Inc. van.
- Provide in-put into the development of treatment plans for persons served to achieve their individual goals.
- Maintain current knowledge of physicians and health-related resources which may assist/or meet the health needs of individuals in the program.
