Pay: $45,000 per year

Day-To-Day
- Provides advocacy in obtaining community services.
- Develops and carries out individualized housing support plans.
- Provides active, intensive, and comprehensive case management services, including accompanying clients to referral sources if necessary.
- Assess and reassess individuals and family needs as circumstances change.
- Maintains client case records in compliance with licensing and agency regulations.
- Assists clients in receiving needed benefits and services within the community.
- Maintains and strengthens effective collaboration with other community agencies and establishes new linkages.
- Collects, reviews, and communicates medical, psychological, educational, social, and employment information and implements needed action under the guidance of the Regional Director.
- Coordinates all communication with referral agencies.
Qualifications
Requires a bachelor’s degree in human services, plus one year of experience working with the homeless population or a combination of education and experience. HMIS experience preferred.
Valid Texas Driver's License, clear MVR, and ability to be insured.
