GENERAL FUNCTION:
The Client Care Manager develops plans and supports clients in setting goals to promote independence and improve quality of life. This role creates plans in alignment with WAC, DDA, and internal policies and procedures. This position involves coordinating and monitoring clients' medical appointments, prescribed medications, and supporting overall health care needs. They support clients through transitions, coordinating with the care teams to ensure continuity of care and minimize disruptions. This position also oversees the hiring and onboarding of new caregivers.
PRINCIPAL ACTIVITIES:
- Partner with clients, guardians, and support teams to create and implement all care plans that align with their Person-Centered Service Plan.
- Obtain and maintain all releases of information.
- Communicates with medical and insurance providers to facilitate coordinated care.
- Responsible for the organization and maintenance of all client records.
- Coordinates with team to ensure healthcare appointments are scheduled and attended.
- Collaborates with Nurse Delegator to ensure they have received communication from teams regarding any changes to delegated tasks and medication.
- Meets with clients and DSPs to discuss client progress toward goals and incorporates any new information or goals into client support plans.
- Works together with the training department to coordinate the initial training of new employees and any client specific training needs.
- Conduct monthly review of client documentation including MARs and charting, and coordinates with the Program Manager to address any discrepancies.
- Attends all annual assessments as part of the client’s care team.
- Attend medical appointments as needed.
- Perform additional duties as assigned.

Qualifications
QUALIFICATIONS:
- Bachelor’s degree in a human services field and at least 1 year experience in a case management or health care setting (relevant experience may substitute for educational requirements).
- Basic understanding of developmental diagnoses and nutrition related health care needs.
- Familiarity with case management and a basic understanding of medication management, commonly prescribed medical devices, and Nurse Delegation laws.
- Proficient in computer operation, with basic knowledge of Microsoft Office Suite.
- Strong organizational skills with the ability to work independently and meet deadlines.
- Exceptional oral and written communication skills.
- Valid Washington State Driver’s license with eligibility for coverage under VOAWW’s vehicle insurance policy, subject to annual review of driving record.
- Must successfully pass and maintain a DSHS background check.
- Commitment to fulfilling ongoing training requirements.
