What are the responsibilities and job description for the Aftercare Counselor position at Volunteers of America?
Essential Duties and Responsibilities
- Complete aftercare calls and sessions for all participants assigned to caseload and currently enrolled in aftercare program
- Coordinate with external providers as requested and / or needed
- Maintain regular contact with participant's family and community workers as necessary pre- and post-discharge; at least monthly to provide resource linkage and referral assistance, as well as other community-based supports as needed and appropriate post-discharge
- Engage in evaluation of discharge recommendations and assist in arrangement of provision of services and referrals as necessary
- Attend and participate in treatment reviews, school meetings, and discharge meetings while in the aftercare program at Bar None
- Facilitate structured, scheduled milieu groups as assigned
- Conduct scheduled individual aftercare planning sessions on a weekly basis with residents nearing discharge and qualifying for the Aftercare program
- Complete transitional services and individual aftercare plan with each resident prior to discharge and update each client's individual aftercare plan every 30-days post-discharge
- Collaborate with other administrative staff to participate in all program outcome measures and evaluation; including any necessary paperwork to be completed with program participants / family for the evaluations study, including and not limited to data collection and qualitative reporting
- Complete all documentation as applicable in a timely manner, including to meet requirements for licensure, accreditation, best practices, and VOA policies
- Attend and participate in assigned weekly meetings, and additional case consults as needed / able
- Comply with agency policies and procedures, state licensing requirements, accreditation standards, and department of health regulations
- Update supervisor as directed on all aspects of work, progress, concerns, and needs of the Aftercare program, and provide creative problem solving as needed
- Assure program participants and employee privacy is maintained by being knowledgeable and following HIPAA privacy and security regulations, and seeking support from supervisor as needed
- Represent the agency in a professional manner at all times
- Work shifts on the milieu as assigned, including 4 weekend shifts per month
- Meet affirmative action goals established by the organization while maintaining a work environment which respects and supports cultural awareness and proficiency
- Communicate effectively, both orally and in writing
- Build and maintain positive working relationships with youth, families, and providers, including and limited to demonstration of appropriate professional boundaries
- Complete other tasks as assigned; all duties are subject to the direction of the supervisor
