JOB SUMMARY
The Case Manager directs and facilitates the delivery of appropriate support services for participant households as indicated by relevant service modalities. The Colorado Rapid Re-Housing for Re-Entry program serves unhoused justice involved individuals and utilizes a housing-first model to assist participants in obtaining stable permanent housing. Job duties include conducting needs-based and program eligibility assessments, engaging in community outreach, providing case management services, and program-wide service coordination. Case Manager is charged with making demonstrable progress towards obtaining and applying knowledge and experience in field-relevant best practices and standards.
Working Conditions and Physical Requirements
- Travel throughout the program's service area is required on a regular basis
- Must be able to work in diverse and, at times, environments such as homeless shelters, service facilities, streets, offices, and all other locations as necessary to fulfill program objectives.
