The Family Coordinator at Vista 44, a family-centered permanent supportive housing project, plays a crucial role in supporting and empowering families transitioning from homelessness / housing insecurity to stable housing. This position involves providing holistic case management services, coordinating support resources, and fostering a safe and inclusive community environment for residents. The Family Coordinator ensures that families receive comprehensive services tailored to their individual needs, promoting self-sufficiency and long-term stability. The Family Coordinator works collaboratively with a multidisciplinary team to create a nurturing and supportive environment that enhances the well-being and quality of life for all residents.
Essentials :
Housing Case Management and Support :
- Conduct thorough assessments of family needs and develop individualized service plans.
- Provide ongoing housing case management, including regular check-ins, goal setting, and progress evaluations.
- Assist families in accessing community resources such as healthcare, education, employment, and childcare services.
- Facilitate referrals to external agencies for specialized services and support.
