We are looking for a Patient Call Center (Triage)'Clinical Support RN to support our locations in Crystal, Maple Grove, Osseo, and Plymouth on a full-time (Monday - Friday'day shift) basis.
The hourly range for this position is $34.00 - $41.00. Compensation is based on years of relevant experience.
Professional Summary:
The Registered Nurse (RN) will work in an ambulatory clinic setting and utilizes a systematic approach to nursing practice incorporating assessment, planning, nursing diagnosis, implementation, and evaluation in accordance with the Nurse Practice Act, recognized professional standards and Voyage Healthcare policy. The RN will collaborate with other health care team members when the need for assistance is identified and plays a critical role in coordination and delegation of care. This role supports and implements patient safety and other safety practices as appropriate. The RN will also maintain professional competencies for practice. Primarily responsible for completing nurse visits, administering immunizations/injections, assisting with procedures, completing patient teaching, and triaging symptom-based walk-in patients that present to the clinic. Previous clinic experience preferred.
Responsibilities:
- Maintain a respectful work environment.
- Provide quality, competent nursing care in a customer friendly manner.
- Receive patient phone calls and following the practice or protocol while gathering pertinent data regarding the reason for the call.
- Provide ambulatory nursing care to patients in accordance with experience, training, and licensure as it pertains to a variety of assignments across several ambulatory clinic areas.
- Implement nursing care according to policies, procedures, and protocols, and in accordance with identified best-practice standards of care.
- Assist with/perform medical procedures and treatments.
- Recognize and respond to medical emergencies and unplanned situations. Perform CPR as needed.
- Provide culturally competent health education and counseling.
- Document all care information, as necessary to maintain an accurate record of nursing actions, patients’ reaction to care, resulting patient outcomes, and relevant medical communication with patients in the Electronic Medical Record.
- Promote a safe, equitable, and respectful environment in which concerns can be addressed effectively.
- Complies with procedures and policies to ensure a safe work environment and compliance with OSHA standards.
- Duties may include, medication and immunization nurse visits, stand-alone RN visits, telephonic and in-person triage, coordination of care and case management, refill prescriptions per protocol or other related duties as assigned.
Requirements:
- Graduate of an accredited school of nursing required
- Registered Nurse (RN) Current licensure as a registered nurse in the State of Minnesota required
- CPR Certification
- Minimum 2 years’ experience preferred
- Excellent communication and interpersonal skills with the ability to understand patients' medical records, physician orders and medication orders to be communicated to the patient and clinic staff
- Strong organization skills and ability to work independently and in a team setting with minimal supervision
- Ability to maintain customer service standards
