Job Description:
Position Title: Enrollment Case Manager (ECM)
Location: Uniontown, PA
Duration: 12 months (C2H-Contract to hire)
Pay: $20.50/hour (fixed; non-negotiable)
Benefits:

Overview:
The ECM will work primarily in applicants' homes, assisting with the HCBS waiver application process. This role requires traveling within and occasionally outside the assigned area to meet business needs. A personal vehicle is mandatory; public transport is not permitted.
Responsibilities:
The ECM will conduct intake visits, assist with application submissions, guide applicants through eligibility processes, and maintain relationships with stakeholders such as community organizations, healthcare providers, and government offices. Regular updates, meetings, and timely management of applications are essential.
Qualifications:
- Associate degree OR at least 12 credits
- At least 2 years of casework experience, including 1 year of functional ability assessments.
- A minimum of 1 year in public or private social work.
This role involves some lifting (up to 25 pounds) and requires excellent organizational and interpersonal skills.
