Enrollment Specialist

Position Details:

NOTE –

• The Enrollment Case Manager will be scheduled (in most instances) throughout the day with appointments that have a travel time of 45 minutes or less from their prior location, usually within their assigned county. All travel expenses will be reimbursed as $0.67/mile.
• The training for the ECM role is virtual, it will be conducted through MS-teams.
• $100 after completion on 120 hours on job and Weekly payment of salary and gas bill.

Job Overview:
ECMs primarily work at applicants' residences, facilitating intake visits for HCBS waiver applicants and supporting eligibility determination. Occasional travel beyond the assigned county may be required. A personal vehicle is mandatory; public transportation is not permitted.

Responsibilities:

• Travel within assigned and neighboring counties to applicants' residences.
• Conduct intake visits, explain HCBS programs, and support eligibility determination.
• Assist with application submission, including data entry in Compass.
• Guide applicants in selecting appropriate MCOs, Life Providers, or Support Coordinators.
• Build relationships with community organizations and stakeholders to improve the application process.
• Collaborate with healthcare providers and facilities to ensure seamless case management.
• Address stalled applications to expedite the process.
• Attend meetings and provide regular availability updates.
• Occasionally handle items up to 25 lbs.
• Perform other duties as assigned.

Qualifications:

• Bachelor’s degree with at least 12 credits in sociology, social welfare, psychology, gerontology, or related fields.
• One year of social work experience.
• Two years of casework experience, including assessments of functional abilities.
• Equivalent education and experience combinations considered.

Job Type: Full-time

Pay: $19.00 - $20.50 per hour

Expected hours: 40 per week

Schedule:

• 8 hour shift
• Monday to Friday
