Job Summary:
The Quality Programs Coordinator will assist the Clinical Performance and Outcomes team in achieving
annual quality measure goals for Heritage Victor Valley Medical Group, including senior and commercial
quality measures. This role involves contacting patients to address quality measure gaps, using customer
service, care coordination, and motivational interviewing techniques to encourage compliance. The
coordinator will also conduct research on patient medical records, document activities in the EMR, and
provide support to physician offices regarding quality measure requirements, including ongoing provider
education.
Responsibilities
- Conduct outreach efforts to HVVMG members to enhance quality measure performance.
Collaborate with HVVMG IPA clinical staff to identify priorities and implement targeted
initiatives, including telephonic or written outreach via mail or the patient portal. - Collaborate interdepartmentally to integrate patient completion of quality measures into overall
care coordination activities. This includes facilitating processes with Quest, QHMO, and
appointment scheduling. - Assist IPA provider staff in delivering quality measure education and training at primary care
offices. Provide regular reports and communication regarding quality measure performance to
ensure alignment with goals. - Support the resolution of quality measure compliance concerns raised by primary care offices,
including direct collaboration with health plans when necessary. - Develop expertise in tools such as QHMO, Manifest MedEx, Coziva, Evoke 360, and Compass to
enhance quality measure performance. - Maintain accurate records of activities in the EMR and q.CALL systems.
The pay range for this position at commencement of employment is expected to be between $21.00 and $24.00/hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience. The total compensation package for this position may also include other elements, including a sign-on bonus, restricted stock units, and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered. Requirements:- Education: High School diploma or equivalent required.
- Experience: At least 1 year experience in a medical group or medical office setting.
- Knowledge Experience with data entry.
- Skills: Ability to utilize computers, computer software/applications, and Microsoft Office
applications (Word, Outlook, Power Point, etc.) with basic to intermediate proficiency. - Skills: Must be neat, accurate and detail oriented.
- Skills: Excellent customer service and telephone etiquette skills.
- Skills: Ability to manage time effectively and prioritize tasks to meet established deadlines.
