The Raleigh House is looking for an experienced and motivated Clinical Discharge Coordinator to join our team!
At the Raleigh House, individualized treatment planning includes personalized discharge planning. Our Clinical Discharge Coordinators are at the heart of this process. We are currently seeking an individual who is talented in the art of motivating clients, building strong relationships with our community partners, and enjoys multi-tasking and organizing details. The ideal candidate has experience in the behavioral health field, enjoys collaboration with clinicians and families, and is tech savvy. This role includes assisting clients with exit surveys, securing appointments, collaboration with the Business Development, Finance and Alumni Teams, and interfacing with our exciting new mobile app.

Essential Functions:
- Meet with clients to discuss their individualized discharge planning.
- Reinforce and support returns to referring partner programs.
- Communicate with partners on the return status of their referents.
- Receive level of care recommendations from clinical team.
- Make calls for bed/seat availability for treatment programs and sober living.
- Conduct interviews with clients as needed.
- Collaborate and interface with the financial team.
- Compile and maintain referral option portfolios for clients to look at and choose programs.
- Coordinate with BD Team on any potential programs that are not currently Raleigh House approved.
- Develop and maintain relationships with referring partners.
- Attend weekly staff meetings.
- Coordinate intake arrangements with Raleigh Houses outpatient program for transfers within Raleigh Houses continuum.
- Develop discharge packets that include continuing care details as well as recovery meetings in the area, and any upcoming appointments.
- Assist clients in securing Primary Care Physicians, if needed, and set 1st appointment.
- Conduct exit surveys with all clients.
- Be accountable for all aspects of discharge are being completed
- Train on intake processes and procedures, and conduct intakes when needed.
Education and Other Requirements:
- Experience with addictions treatment in an outpatient setting, strongly preferred but not required.
- Must possess a valid drivers license and have a reliable form of transportation.

- Experience with treatment facilities and/or addictions and mental health populations preferred.
- Prohibitions: Active addiction, use of illicit substances (including THC) during employment, current sponsorship of a current or former (less than 1-year post RH treatment) client.
- Must undergo a pre employment drug screen.
