Job Title : Licensed Practical Nurse (LPN)

Position Summary :
Seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Assertive Community Treatment (ACT) team. This role is central to our trauma-informed care approach, ensuring that all clients receive the best possible care within a supportive and healing environment. Key Responsibilities :
Provide compassionate nursing care to clients, integrating trauma-informed practices into every interaction.
Create a safe and supportive environment that fosters empowerment, resilience, and healing.
Collaborate with other team members to develop and implement individualized care plans that incorporate trauma-informed care principles.
Assist with assessments and monitor patients' physical and mental health conditions, documenting any changes and communicating with the ACT team.
Administer medications and treatments as prescribed, while ensuring that clients are supported throughout their recovery journey.
Engage with clients to promote understanding of their care plans and offer education on health-related topics.
Offer emotional support to clients and their families, understanding the emotional and psychological effects of trauma.
Ensure compliance with all health, safety, and regulatory guidelines.
Participate in team meetings and contribute to the development of treatment strategies for clients.
Qualifications :
Current Oregon Licensed Practical Nurse (LPN) license in good standing.
Experience in mental health nursing or a strong interest in trauma-informed care practices.
Knowledge and experience working with individuals who have experienced trauma, with an emphasis on mental health and recovery.
Excellent communication and interpersonal skills for working collaboratively with the multidisciplinary team and engaging with clients.
Ability to work independently and as part of a team, demonstrating empathy, respect, and professionalism.
Strong documentation and organizational skills.
