Job Summary:
The Member Care Coordinator is responsible for conducting home health assessments, educating members on healthcare programs, and ensuring adherence to personalized care plans. This role involves outreach, data entry, member education, and supporting the clinical team in care coordination.
Key Responsibilities:
Perform home health assessments and update member records.
Conduct outreach and follow-up to ensure member engagement with care plans.
Educate members on programs, services, and schedule appointments.
Monitor and support members’ progress toward care plan goals.
Assist clinical teams with non-clinical functions and case management.
Respond to member inquiries and process information accurately.
Ensure compliance with HIPAA and corporate policies.
Qualifications:
LVN/LPN, RN, or equivalent experience in managed care (3 years of relevant experience).
Behavioral health experience preferred.
Knowledge of medical terminology and care coordination.
Proficient in Microsoft Office and data entry.
Strong communication, customer service, and motivational interviewing skills.
Valid state driver’s license, reliable transportation, and applicable insurance.
Willingness to travel as needed.
