Responsible for providing job development, training, and on-going support for adults with special needs seeking and being placed in community employment. Provides support to worker, family, employer and other service providers to ensure successful worker adjustment and performance. Responsible for the direct supervision of individuals served.
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
- Responsible for interviewing individuals to assess knowledge, skills and abilities using pertinent collateral information, including information from other service providers.
- Provide job search assistance and information to individuals including visiting potential employers, application process, arranging interviews and resolving employment barriers.
- Perform job site analysis, which includes identifying specific job skills, accommodations and working environment to provide appropriate job match and placement of individual served.
- Perform and compose job task analysis sufficient to provide ongoing training of the individual served in the work setting and to provide periodic back up coverage.
- Develop and maintain good working relationships and effective communication with individual's supervisors, co-workers and any other team members.
- Maintain billable time requirements.
- Provide support as need to individuals placed in community employment jobs to assist them in adjusting and maintaining their employment.
- Monitor individuals at a minimum of two times per month at the job site specific to provider standards and more if needed.
- Job development with potential employers.
- Prepare reports on the individual’s performance.
- Record attendance logs and individual’s time sheets.
- Prepare narratives complete with goals.
- Submit all documentation according to deadlines established by program procedures and provider standard timelines.
- Must sign a Non- solicitation agreement.
- Maintain regular attendance and punctuality and accurately maintain attendance records.
- Maintain confidentiality, integrity, and trustworthiness.
- Must provide own transportation, have a valid driver’s license, registration and proof of insurance and acceptable driving record.
- Assist the immediate supervisor with various administrative tasks to support department operations as directed; may also be assigned special projects.
- Maintain a flexible schedule is required to meet the demands of the consumer's employment objectives.
