Job description

As a recognized Hemophilia Treatment Center and a Center of Excellence, the Washington Institute for Coagulation d/b/a Washington Center for Bleeding Disorders, is committed to the wellbeing of patients with an inherited bleeding disorder across the State of Washington, neighboring states, and Alaska.

WACBD is searching for a dynamic Clinical Pharmacist to support all parts of our pharmacy operations. This is a 32 hour per week position that is eligible for regular employee benefits. More hours may be available after orientation.

POSITION SUMMARY:

The Clinical Pharmacist assumes day to day operational functions of the Pharmacy. Responsible for coordinating daily clinical and operational activities of WACBD pharmacy programs, in accordance with WACBD policies and procedures and requirements of State and Federal regulatory agencies. Responsible for coordination of pharmacy technical staff and clinical pharmacy service programs. The pharmacist, proficient in the required skills and competencies, is responsible for monitoring drug therapy, providing drug information, consulting with medical, nursing, and allied healthcare professionals to deliver high quality, cost effective pharmaceutical services.

Provides support to pharmacy department team to formulate and implement policies and plans to meet the WACBD’s short- and long-term objectives.

Coordinates with various departments such as clinic staff and with other allied health providers to fulfill duties and functions and provide patient care support activities. Assumes full or partial responsibility for plans and projects designed to support the WACBD’s mission and vision. Attends clinical rounds, staff meetings and appropriate training seminars and workshops as required.

Performs related duties as required.

Essential Responsibilities & Functions:

Leadership & Coordination:

  • Coordinates day to day department activities in accordance with established WACBD policies and procedures and accepted standards of practice, and Federal and State laws.
  • Ensures compliance in areas of responsibility required by, but not limited to, department policies and procedures, the Washington State Board of Pharmacy, Federal regulations, HIPPA, CMS and 340B.
  • Identifies areas needing change and provides creative solutions for clinical and/or system problems which impact patient care or service.
  • Serves as a liaison between the pharmacy department, and the nursing, medical, and allied healthcare staff of WACBD and its’ affiliate organizations (MBCC, SCH, PSH).
  • Serves as a patient advocate with payers and community providers.
  • Facilitates positive interactions and professional support for partners in the care of the patient (i.e., Mary Bridge Children’s Center, Seattle Children’s Hospital and Providence Sacred Heart Hospital in Spokane).
  • Ensures customer and patient needs and expectations are met.

Operational Duties:

  • Assists the Pharmacy Director in developing, implementing, and administering plans and programs for short- and long-range goals and objectives.
  • Develops or revises policies and procedures required for efficient departmental clinical services.
  • Ensures accreditation readiness and ongoing support of practices necessary to maintain accreditation.
  • Assists with pharmacy operational issues, such as weekly inventory, and the maintenance of pharmacy records in compliance with, but not limited to, the Washington State Board of Pharmacy, Federal regulations, HIPPA, CMS and 340B.
  • Assists with training and evaluation of departmental staff.
  • Manages the procurement, storage, preparation, dispensing, recording, and controlling of all drugs, including investigational drugs within the department.
  • Utilizes the software system for prescription processing. (i.e., updating and maintaining all data files)
  • Generates required reports- daily dispensing records, delivery tickets, inventory, third party payer, A/R reconciliation.

Clinical Pharmacist Activities

  • Performs clinical pharmacy functions accurately and in a timely manner. These functions include an assessment of all aspects of medication related therapy necessary to provide for the safety, efficacy, and cost-effectiveness of drug use.
  • Provides clinical services that optimize the delivery of pharmaceutical care including but not limited to, the regulation of drug therapy, provision of drug information, prevention of drug interactions and adverse drug events, screening for drug allergies, evaluating and monitoring drug therapy.
  • Documents clinical pharmacist activities and patient communication in the patient management platform.
  • Participates with treatment and transitional care planning with patients and providers.
  • Recognizes and addresses the age-related needs of the population served.
  • Participates in rounds with medical, nursing and allied health providers.
  • Provides accurate dosing information to all practitioners and hospital customers, and counsels patients and families about the proper use of each medication dispensed in accordance with State and Federal regulations.
  • Reviews on call/after-hours patient care needs, including delivery tracking and any immediate efforts to support patient care.
  • Participates in WACBD pharmacy Quality Improvement projects.
  • Assists in the implementation and administration of plans and programs, participates on committees as needed.

Distribution Activities:

  • Processes and accurately interprets all prescriptions in a timely manner.
  • Reviews all orders for appropriateness and accuracy.
  • Performs the drug distribution procedures necessary to ensure- five rights. -right patient, right drug, right dose, right route, right time.
  • Follows all applicable procedures in dispensing and compounding medications. Completes all required paperwork accurately and in a timely manner.
  • Assures compliance with all WACBD policies as well as State and Federal regulations.
  • Utilizes good communication skills in handling all issues.
  • Serves the wholesale business and daily wholesale distributor activities.

Educational Activities:

  • Provides in-service programs to physicians, residents, nurses, pharmacists, and other allied health professionals.
  • Acts as a preceptor to pharmacy students and pharmacy technicians in training. All preceptor roles shall follow State and Federal regulations and licensing requirements.
  • Provides education and consultation to hospital customers seeking guidance on treatments for bleeding disorders, factor dosing, storage, handling, reconstitution, and administration.
  • Maintains and shares knowledge of current treatments in bleeding disorders with other pharmacists and staff.
  • Participates in regional and national meetings and conferences

Qualifications:

Education/Experience/Skills:

Typically, these skills and experience result from a Pharmacy degree:

  • BS or PharmD in Pharmacy from an accredited college or university
  • A current Washington license as a registered pharmacist
  • Experience working in a collaborative team setting is required.
  • Specialty pharmacy experience is preferred

Competencies:

To perform effectively in this position, the employee must have:

  • Excellent interpersonal, verbal, and written communication skills
  • Thorough knowledge of State and Federal laws.
  • Analytical skills to determine the appropriateness of drug therapy and the correctness of the dosage, and be able to evaluate drug interactions, ADEs and allergic reactions.
  • Skills and experience necessary to perform system analysis in support of quality improvement.
  • Organizational skills necessary to coordinate departmental and interdepartmental activities.
  • Thorough knowledge of current trends in managed care and third-party reimbursement.
  • Thorough knowledge of drug therapy in the pediatric and adult population served.
  • Knowledge of word processing, spread sheets, data analysis and reports.


Job Type: Full-time


Pay: $58.70 - $67.00 per hour
