About The Role

Minimum Qualifications

  • Current RN or LPN license in the state of Oklahoma.
  • Associate degree in nursing or related field (for RN) or an Vocational or Technical degree in nursing (for LPN).
  • Previous experience in case management or a related healthcare setting.

Preferred Qualifications

  • Experience with electronic health records (EHR) systems.
  • Knowledge of community resources and healthcare regulations.
  • BLS

Responsibilities

  • Conduct comprehensive assessments of patients' medical, psychological, and social needs.
  • Develop, implement, and evaluate individualized care plans in collaboration with healthcare teams.
  • Coordinate services and resources to ensure continuity of care for patients.
  • Monitor patient progress and adjust care plans as needed to achieve desired health outcomes.
  • Educate patients and families about health management, available resources, and care options.
